I understand that I will have to pay $1045 in seven non-refundable installments:  $100 deposit to hold students spot due May 25, 2017; $175 due October 2, 2017; $175 due January 5, 2018;  $175 due April 6, 2018;  $100 due May 22, 2018:$175.00 due August 31, 2018 $Final Balance due October 1, 2018 otherwise my student(s) will not be permitted to attend.  Students are responsible for providing their own spending money.  Cash or Checks made payable to WYHS Choirs.  I understand the trip is an investment and is NON-REFUNDABLE unless I purchase the Travel Insured International trip insurance package for $45 and agree with the insurance policies and agreement *

I give my student permission to attend the WYHS Disney Trip 2018 with the understanding that if my student receives any out of school suspensions/expulsions, moves out of the district or quits the choir program they will no longer be eligible to attend our trip.  Students should be trustworthy in school in order to travel with the school out of state.  (Please SIGN/DATE electronically below parent/guardian ONLY) *
