Waterford Dance Academy Online Registration Form

Online Registrations for Waterford students are charged to student accounts. A printable form is available if you would prefer to send a check. To register in person, please see Kate Linsley or email katelinsley@waterfordschool.org.

If you are a parent of a student who does not attend Waterford, you can still use this form. See the Acknowledgement section below.

A nonrefundable application fee of $25.00 is required with this application for each initial enrollment. All school policies, student codes of conduct and contractual enrollment agreements made in the 201-2016 school year are applicable and binding. Families with multiple children enrolled can take a 10% discount off of their total course fees.

You may sign up for individual terms or for the entire year. Academy fees will be billed near the beginning of each term.

I have read and agree to the terms above, as well as the Dance Academy Code of Conduct. Please register my student for this course and bill the fees to my account. A $25 application fee is charged with your first application of the year, and will be added to the total amount. Parents of students who do not attend Waterford can mail a check to the Waterford Dance Academy, 1480 E 9400 S, Sandy, UT 84093, or drop off check at the Dance Office, room 812A.
