2012 Viodi Content Awards Early Bird Submission Form
Please submit one form per title submitted. In general, links to 30 second trailers will be required for submission and not an entire program.  We also request a brief write-up explaining the impact of a particular submission on the community (e.g. we saw donations rise 50% after airing a documentary about a local non-profit). By submitting your (content title), the submitter warrants that he or she has the right to submit and exhibit the content in any media formats and through any media channels and will indemnify and hold harmless Viodi, LLC, its directors, staff, contractors and sponsors, and any media channels where the content may be exhibited against any and all claims arising out of the exhibition and promotion of said content..  Viodi hereby has my permission to use my company’s name, video(s), photos, and press materials and any derivative works for pre- or post-Content Awards promotion.  All other terms are provided in Viodi’s standard terms and conditions, which can be found at http://www.viodi.com/5-02terms.pdf. The person signing this document expressly represents and warrants to Viodi, LLC that he or she is authorized by their company to submit this content and to sign this agreement.
