Growing Hope Volunteer Registration Form
I acknowledge that I have volunteered to participate without pay at the Growing Hope Center site and in other activities with Growing Hope. I am aware that work such as gardening and construction may be a hazardous activity. I am voluntarily participating in the activities, which include, but are not limited to, loading and unloading materials, planting and gardening, using hand tools and power tools, with the knowledge of the danger involved. I also understand that I have the right to refuse any task that I am uncomfortable completing.
In consideration of being granted the opportunity to volunteer with Growing Hope, I release and forever discharge Growing Hope and its Board of Directors, administrators, employees, agents, assigns, officers, and any cooperating landowners and partner organizations for all claims and damages, demands, and any actions whatsoever, including those based on negligence, in any manner arising out of my participation in this activity. I understand that this release means that, among other things, I am giving up my right to sue Growing Hope or its partners for any such losses, damages, injury, or costs that I may incur. I am duly aware of the risks and hazards that may arise through participation in Growing Hope's projects and assume any expenses and liabilities I incur in the event of an accident, illness, or other incapacity.
By filling out this form, I agree that I have read the above statement and the following volunteer guidelines, and that I have discussed any questions I might have with a Growing Hope staff member. I hereby agree to accept any and all risk of injury, illness, or death, and verify this statement by placing my initials at the end of this form.
o Volunteers will do their best to keep themselves and other volunteers safe and healthy while volunteering (e.g., asking for help, drinking enough water, taking breaks, using tools safely).

o Volunteers will be respectful of other participants, volunteers, employees, and community members while volunteering with Growing Hope
o Volunteers will use good common sense to make decisions and will ask for clarification from Growing Hope staff or supervising volunteers.
