This form is to request how to display the name on the program, diploma and where it should be mailed. The name information must match what is listed in HawkWeb. Name changes are completed in Admissions with supporting documentation and only accepted before the beginning of each semester.
THIS FORM MUST BE SUBMITTED BY NOVEMBER 30, 2018 by 5:00 PM for FALL 2018 and APRIL 26, 2019 by 5:00 PM for Spring 2019. DIPLOMAS WILL BE HELD IF THE FORM IS NOT COMPLETED.
