This is the Practical Nursing Program Application for UofA Hope Campus Day Program 2017-2018. SUBMISSION DEADLINE: MAY 1, 2017. This application is good from March 1, 2017 through May 1, 2017. Program start date is July 5, 2017. Completion May 10th, 2018. READ APPLICATION CAREFULLY!!! Answer all questions with complete information. Schedule pre-entry test (CAT test) and Register for your background check at end of application.

    If you attended another learning institution, you MUST submit an unofficial transcript -COPY to the nursing department. All college transcripts should be submitted from all colleges you attended. Failure to do so may result in an ineligible application. Mail, Fax, or Hand Deliver to practical nursing department. We recommend bringing a copy to your CAT testing appointment. IN ADDITION, an OFFICIAL transcript should be mailed from previous school/college to "Enrollment Services" on this campus. Send "ATTENTION: Registrar".

    Arkansas State Board of Nursing

    The Arkansas State Board of Nursing denies the application for the National Council Licensure Examination ‐ Practical Nurse (NCLEX‐PN®) of nursing program graduates who have plead guilty, nolo contendere, or been found guilty of certain offenses as identified in Arkansas Code, Act 1208 of 1999 Legislative Session ‐ ACA 17‐87‐312. Some offenses result in denial of licensure exam application (after graduation from the nursing program) but others are investigated by the board and application MAY OR MAY NOT be approved. ALL offenses must be reported even if a suspended imposition of sentence occurred, if the applicant was a juvenile during the offense, and/or if the criminal record was sealed or expunged because the state board of nursing can see these offenses even if the applicant thinks they are no longer a part of their record. Prior to acceptance into the nursing program, a criminal background check must be performed. A student who has a criminal history of any of the offenses that prevent licensure examination approval is not eligible to enter or to enter or remain in the nursing program and will either not be accepted or if results are returned after acceptance into the program the student will be required to withdraw. Many clinical facilities also deny a student from performing clinical rotations there if certain offenses have been committed. Some of these facilities perform their own background check on the students and some require that the program provide them with the student’s criminal background check results. If a student is denied approval for clinical rotation in any of the facilities that are utilized for the clinical practicum courses, that student is no longer eligible to remain in the nursing program and will be required to withdraw due to inability to complete program requirements.

    All Applicants must complete the Cognitive Abilities Test (CAT test) to be considered for the nursing program.

    Select a date below to take your CAT test. VERY IMPORTANT - - REMEMBER your testing date!!! Bring $10 cash or money order to pay for you exam. Print and bring an unofficial COPY of ALL transcript(s). If later you realize you can't make it to the test date selected then you may show up to any of the dates listed below. However all testing must be completed by LAST offered EXAM DATE of May 2nd. The CAT TEST may only be taken once in 12 month period. You will be ineligible if exam repeated before 12 month period.     FUNCTIONAL ABILITIES STATEMENT

    Nurses must possess knowledge, application of skills, and abilities in order to provide safe and effective nursing care. Functional Abilities are those specific abilities/attributes a nurse must possess in order to practice safely and effectively. The National Council of State Boards of Nursing (NCSBN) identified and defined categories of functional abilities (see below). The functional abilities for safe nursing practice include fine and gross motor skills, physical endurance and strength, mobility, hearing, visual, tactile, smell, reading, arithmetic competence, emotional stability, analytical thinking, critical thinking, interpersonal skills and communication skills. Some limitations in ability can be accommodated with special devices or special circumstances; others cannot. The reasonableness of accommodations will be based upon client safety and the resources of the College and department. Temporary or newly diagnosed interruptions in abilities (i.e. casted hand) will require individual evaluation.
