By completing the form below, you are asking the Office of Teacher Education to create a voucher that will cover some or all of your course costs for the semester. Please complete this form prior to the first day of the semester in order to avoid incurring late fees. If you are indeed eligible for a CT voucher, OTE will send it to the bursar on your behalf. If you do not appear to be eligible for a voucher, OTE will reach out to you.
Important Notes:
1) Your student teacher must have submitted his/her Record of Hours form before we can process your request
2) If you are eligible for a voucher, tuition credits will be posted to your account by the bursar's office approximately 2 weeks AFTER the start of the semester.
