Request for Policy Exception for a Student Issue

This form is to be used to request an exception to an established policy of SCU related to student issues. All requests must be submitted at least seven (7) days prior to required action deadline. Once this request is received by the registrar's office, it will be routed to the appropriate personnel from the following list, depending on the policy to which the request is related and who governs the policy: Department Supervisor/Chair, Dean (academic/student life issues ONLY), Council/Committee Chair that administrates the policy, and the Cabinet Member that oversees the policy and has final approval authorization.
