Teacher Recommendation Form - Choir Auditions
Hello teachers! Thank you for taking the time to fill out this recommendation form for my auditioning choir students. Being a part of an auditioned choir at EHS requires the ability to balance academics, athletics, and other extracurriculars, as well being passionate, encouraging, hard-working, positive, and working well with others. I am looking for more than talent to be a part of my top choirs. You see these students in a different light than I do, and it is important to me that I choose students to participate in my auditioned choirs that have these traits. Please fill out this questionnaire honestly - everything is confidential!

I need to receive all recommendations by Friday, February 10th! Feel free to contact me with any questions, and thank you in advance!
