Library Instruction Request Form
Librarians instill information literacy skills in SUNY Sullivan students through a program of course-integrated instruction, tailored to the specific needs of a particular class.

At the Hermann Memorial Library we are dedicated to teaching our students to be information literate in the class room and everyday life. We strive to create active learning opportunities that are student centered and go beyond the traditional lecture and demo.

Additionally, instruction librarians are available to collaborate on assignment design.

Instruction sessions or assignment assistance can be requested in several ways:

1. Complete and submit the online form below.

2. Contact Greg Carr, Public Services Librarian, by email at gcarr@sunysullivan.edu or phone 845.434.5750; ext.4227

3. Call the Library's Information Desk at 845.434.5750; ext.4389
