So your Sunskis are busted, but we've got you covered! After you fill out this form and if you have been instructed to send in your shades, we will receive your information and await your damaged shades to arrive at our office. Please allow at least 2 weeks for us to get to your repair. It's just a small crew of us at Sunski after all!

If you have not been instructed to send in your shades and are just in need of a replacement, please fill out the form and make sure you give us the correct code on you shades, or, if you cannot find it, the complete name of your shades. For Example, "Dipsea" does not suffice, but "Dipsea Tortoise Amber" does, and so does "SUN-DS-TAM"

What is the code that is on the right arm of the sunglasses frame? (looks like SUN-XX-XXX). *
It will read SUN-XX-XXX (please type it in that format). If there is no code or it has worn off, please tell us the name of your Sunskis and their color. If you don't tell us the color or style we have no idea what to send you! Please refer to our Shop page on our website to find your Sunskis if you cannot remember their name. If your style of shades is sold out, then please let us know which style you would like instead.
