After you fill out this form and if you have been instructed to send in your shades, we will receive your information and await your damaged shades to arrive at our office. Please allow at least 2 weeks for us to get to your repair. It's just a small crew of us at Sunski after all!
If you have not been instructed to send in your shades and are just in need of a replacement, please fill out the form and make sure you give us the correct code on you shades, or, if you cannot find it, the complete name of your shades. For Example, "Dipsea" does not suffice, but "Dipsea Tortoise Amber" does, and so does "SUN-DS-TAM"
