Forfeits must be notified using this form by 6PM on the day before the game.

- It is the responsibility of the team Forfeiting to use this form to notify the
  opposition team and umpires.
- A team failing to use this form may be considered as not have given the
  required notice and may be subject to a fine and/or points loss.
- Any Umpiring or Grounds/Canteen Duties will still need to be performed or a fine may apply.
- This form will be sent to the Opposition Club, Umpires Club, Competition Convenor
  various committee positions and the Association Secretary automatically via
  email when submitted

NOTE: Any Forfeit Notified is irreversible (even if the round is rained out and replayed later)
            so be sure this is what you want to do.
