Emergency Relocation Form
Please use this form to provide details of relocatable staff. A relocatable staff is defined as a staff member that is not from the locality from which people are being evacuated and may include national and international staff members. To avoid any delays during a relocation please ensure the staff member is carrying the ID used in this form.

One form should be submitted per staff member. Please use only one relocation focal point per organisation. Before any relocation we will confirm the responses with the designated focal point.
