Your NHS wants an appointment with you!
We are the Hertsmere locality of NHS Herts Valleys Clinical Commissioning Group (CCG). Our members are local GPs in Bushey, Radlett, Borehamwood and Potters Bar and we are responsible for planning and monitoring the quality of health services in West Herts.
We need people who are able to give a small amount of time to meet with other patients and give their views on local health services and help with plans for the future.
If you would like to become a member of the patient community who support your GP practice and are consulted on the services they provide, your practice has a patient participation group you can join – just ask at reception, look at their website or give them a ring for details.
If you would like to get involved in wider health services such as hospitals; clinics; community, mental health and learning disability services; pharmacists; district nurses to name a few then there is a local Hertsmere patient group you could join. If you are interested in finding out more, then contact
In the meantime we would like to ask a few questions that will help us make sure that the Hertsmere patient group is as accessible as possible:
Q1: Would you like to have a say in your local health services?
Q2: What way would you like to be involved? Select any that apply
By joining the patient participation group at my GP practice
By joining the Hertsmere patient group
Contact by email
I don't wish to be involved
Q3: If you would like to join the Hertsmere patient group, what would be your preferred time during the week for the group to meet?
Q4: Some questions about you (optional). Which GP practice are you registered with?
Q5: So that we can keep you informed, what are your contact details - preferable email address
Q6: Are there any areas of health services that you are particularly interested in?
