OMHS 2017 FANS Camp Registration
July 21, 2017 at OMHS from 10:00 A.M. until 2:00 P.M.
This form along with payment must be submitted in order to be registered for FANS Camp. Payment can be submitted to the OMMS or OMHS main office. The registration fee is $25. If paying with check, please indicate the name of the student on the check. If you have any questions, please contact one of the following:

**Please note that if a student has specific food allergies, the student will need to bring a lunch with him/her to FANS Camp.
