Instructions for completing form
This short form has been developed as a voluntary and cooperative effort between the Foundation for Service Dog Support, Inc., and Handi-Dogs, Inc. The purpose of this project is to gain a clearer understanding of the numbers and types of public access challenges that are currently being experienced by service dog teams.

Please complete this form to the best of your ability. If a question does not have a choice that exactly describes your situation, please select the option that most closely describes your situation. The responses are anonymous, and we will monitor these responses to collect aggregate data. This data will be used to drive recommendations for changes, as well as public education efforts designed to improve the lives of service dog teams.
