We prefer you report your child's absence online, by filling out this form, but if you need to call, please dial our main office line and listen for the prompts: (847) 398.4288

PLEASE NOTE:

  • If you are reporting a student who is arriving late or leaving early, please call our school office at 847.398.4288 instead of completing this form.
  • This form will be checked between 6:45 AM and 9:00 AM each day. After 9:00 AM, please call the school office.

LEGAL NOTICE: This form is to be filled out and submitted only by the student's parent or legal guardian. In the event that a person other than the student's parent or legal guardian fills out and submits this form, AHSD25 reserves the right to take any legal action for fraudulent submission and/or take disciplinary action against any AHSD25 student(s) involved. We reserve the right to call home and verify the absence as needed.
