Online Sports Registration Form
To register for a sport at Grace Lutheran fill out the questions below.  You will need to fill out a separate form for each sport and child  that you are registering.  After submitting your registration form you will need to send in the sports fee and student's physical form to the office. Parents with more then one child participating may include all registration fees on one check (make sure to write the sport in the memo line).  Please DO NOT include THIS payment with any other school charges.  Registration, Payment and Physical Form need to be received BEFORE student may participate.  (Late fees will be added when items are not turned in within the set time limits).
