Fall 2015 Tutoring Registration Form
Fall 2015 policies and procedures WILL deviate some from the recent past. These changes are anticipated to be released by June 20th, 2015. As of now we will continue to ask for the following: 48-hours notice for all schedule changes, all students are required to commit to a minimum of 4-hours of tutoring a month (with the exception of September - with only a 3-hour minimum), students pay for tutoring a month in advance, and all additional hours of tutoring will be billed the subsequent month. In other words January payment will be paid the first week of December and February payment will be due the first week of January. If a student meets more than the pre-determined amount, those meeting times will be billed on the subsequent month's invoice.
