Required
If you included 'Exotics', what type of exotic animals do you have experience with?
What type and how many animals do you currently have in your home? *
Put N/A if this question is not applicable to you
If you do not currently own an animal, have you ever owned an animal?
Clear selection
Are the animals currently in your home up to date on their vaccinations? *
Are the animals currently in your home spayed or neutered? *
All dogs and cats 6 months or older in the home must be spayed or neutered unless there are medical reasons for not doing so.
If you own cats, do you allow them outside? *
If you own dogs, do they regularly interact with other dogs that are not in your household? *
For example, at the dog park
What is your vet's name, phone number, and the name of the owner and animal for all animals in the household (e.g. also for the pets of any roommates).  This needs to be the vet that can confirm your animals are current on all vaccinations and are spayed/neutered. *
Please also include the information for any past veterinarians. Please contact the veterinarian's office and give them permission to talk to RCHS (many vet offices require this permission before releasing any information).
Do you own or rent your home? *
If you rent, what is your landlord's name and phone number? *
If you do not rent, please put N/A in the box.
Do you have a fully fenced yard? *
This will only be a factor if you wish to foster dogs; if you do not have a fenced yard, it will not keep you from fostering and will simply help us determine what foster is a good fit for your home.
Please describe the general area where the animals you are fostering will be kept *
Do you have children, and if so, what are their ages? *
Is anyone in your household currently fostering for another animal organization? *
Have you ever fostered animals before? *
This applies to any organization
Do you agree to allow a RCHS representative to perform an initial home check? *
The representative will need to see the areas of the home where the foster animal will be allowed and also visit with family members and current pets
Are you willing to allow RCHS to visit the foster animal(s) at your home? *
For example, to take digital photos for Petfinder.com, to administer vaccines, etc.
Are you willing to abide by all animal control laws with regard to your foster animals? *
Can a prospective adopter call you regarding your foster animal? *
Frequently, we try to have animals adopted directly from a foster home.
Can a prospective adopter visit your home to see the animal? *

Additional Information
Please include any additional information you would like us to consider while evaluating your application.
You can also include any questions or comments you may have regarding fostering.
By submitting this Foster Care Application you acknowledge that you will abide by the provisions below:
Foster dog(s) must be kept on a leash or enclosed (within a fence adequate for the animal, or in a home) at all times.

Foster cats must be kept indoors at all times.

Foster parents agree to house the animal indoors in a facility that is sufficiently heated and cooled where the ambient temperature does not fall below 45 degrees or go above 85 degrees for more than 4 consecutive hours.  They also agree to keep the facility adequately clean and ventilated at all times.

Foster homes are considered to be an extension of RCHS and are covered under our license.  We are required to share information on each foster home with the Kansas Department of Agriculture.

A foster animal(s) may be in your care for 1 week to 3 months…or more.  If you know that will be out of town for a period of time, you must inform RCHS early enough to allow us the time to find the most suitable temporary accommodations for your animal.

RCHS will be responsible for any veterinary bills. As a foster parent, you will be responsible for advising RCHS of needed veterinary care. All veterinary appointments must have the prior approval of a RCHS cat, dog, or exotics coordinator.

All new foster parents will be advised and given adequate direction by RCHS before an animal is brought into the home.  While you are caring for foster animals, RCHS may contact you from time to time to check on the animal(s)’ progress and address any concerns you may have. It is important that you communicate in a timely manner with the foster coordinators and let them know right away if you run into any issues.

As a foster parent, you will be expected to keep in touch with the animal coordinators about how your foster animal is doing and provide information about the animal for RCHS to post on Petfinder and other social media sites.

RCHS will be available to answer any questions or concerns you may have about your foster animal.
