Quidditch Canada Injury Report Submission 2016-2017

Your participation in this submission form is optional but strongly encouraged.

All questions are voluntary, as is the level of detail you choose to provide. This information may be used by Quidditch Canada to inform gameplay decisions and policies, as well as for insurance purposes. All information is anonymous and will remain anonymous. It will be distributed or released to third parties only in aggregate form. In order to ensure that only one report is submitted per injury, we ask that submissions be made only by the injured player themselves or by the injured player's captain on their behalf.

If you would like to submit an insurance request as a result of an injury incurred at the practice, game, tournament, or event hosted by Quidditch Canada or a Quidditch Canada member team, you may do so by contacting our Membership Director at jill.staniec@quidditchcanada.com to obtain the appropriate form.

Quidditch Canada strives to provide its members with a safe and fun atmosphere for competition. Suggestions for improvement to our rules and policies are welcome at any time and can be directed either to the appropriate department or to info@quidditchcanada.com.
