PLEASE COMPLETE THIS FORM BY MONDAY 29 MAY 2017

INTRODUCTION
QPS prides itself by offering enhanced learning opportunities for all students. The Snowsports Programme is part of our Education Outside the Classroom (EOTC) and is one of these learning experiences.

Your annual parent contribution covers the cost of this activity for your child. Parent/caregiver financial contributions are essential for QPS to be able to provide this activity to students. Contributions can be made via cash, eftpos or cheque at the school office, or by automatic payment to Queenstown Primary School’s bank account 03 0675 032914600 referencing your child’s name.

Should you have any concerns regarding contributions for the Snowsports programme please contact June Sumner admin@queenstown.school.nz or phone 4429120.

There are two payment components to the programme:
1) The skiing and snowboarding programme is included in your parent contribution. This includes transport to and from Coronet Peak, rental equipment and a 1.5 hour ski/board lesson for each of the 6 days. Please note the programme cost remains the same whether you have your own equipment or not.
2) The purchase of a $100 season pass. This is payable to NZSki.

SEASON PASS PURCHASE – payable to NZSki
All Year 3 - 8 students will participate in the QPS Snowsports Programme and will require a season pass at $100 per pass.
If your child/ren has had a pass previously they do not need to be with you at the Snow Centre. If this is your first season pass purchase then your child/ren will need to be with you to have their photo taken.
Passes will be processed at the NZSki Snow Centre on Duke Street from 8 May 2017. NB: There is no season pass form to be completed this year.
