Facilities Request Form
This Facilities Request Form is for Heritage internal use only. All events must be approved by Athletic Director and/or Plant Manager. All fundraising events must also be approved by ASB. Submitting this request form does NOT guarantee the dates and times requested. Requests should be submitted a minimum of 2 weeks prior to the event. Standard requests usually take 2-3 business days to process. Special requests may take 5+ business days to process. Advisor/group is responsible for their own technology hardware (i.e., laptop, projector, sound system).
