Application to Offer District-approved Training
Provo City School District
This Application consists of 3 parts:
1. Basic Course Information (required)
2. District Standards for Professional Learning (required)
3. UBOE Course Credit Application (recommended)

(Those hoping to offer district-approved Lane Change credit should see the value in offering UBOE credit for their course and, whenever possible, should use this application to apply for both. More information about this is available at the end of this survey.)

Criteria for Obtaining District Approval
The PCSD Instructional Council reviews applications for the right to offer district-approved Lane Change credit for participation in various professional learning activities.

[If your course does not meet the above criteria...]
Teachers can accumulate re-licensure points in many other ways. (See “Categories of Acceptable Activities for License Renewal,” Utah Administrative Code R277-500-5.)
