Shining Star Award Nomination Form
Presented twice annually to a Perrysburg Schools employee who is nominated by his/her colleagues as a Shining Star of our district.

A panel representing a cross-section of the district will choose each award recipient from the pool of nominees that has come in since the last award was given. Nominees must be resubmitted for consideration in each round.

Nomination Deadlines:
October 1 for recognition at the regular November Board of Education Meeting
April 1 for recognition at the regular May Board of Education Meeting
