This form was created to streamline the late work process so students could easily share with Zizmor if an assignment is going to be late for any reason. Consider this form your prearrangement for permission for late work.  In order for your work to be accepted you must fill out this form for each assignment  that will be late whether you plan to use a late pass or not.  Remember you get one week past the due date to turn in the assignment and without a late pass it will receive only half credit.

If the student has a late work accommodation as per IEP or 504 then the assignment will be allowed a 4 day grace period to be completed.  After the 4 day grace period it will be considered late and will suffer the same scrutiny as other students in the class (can be turned in within a week of the due date for full credit with a late pass otherwise it will receive 50% credit for up to two weeks after the due date).
