Payment Information

You must pay the $150 deposit for us to hold your place in the program. Deposits are non-refundable. You can pay for the deposit by choosing one of the following options: *Mailing a personal check made payable to SUNY Oswego (Puerto Rico only) *Mailing a banker's check made payable to SUNY Oswego (all other countries) *Mailing a postal money order made payable to SUNY Oswego Deposits can be mailed to: SUNY Oswego Division of Extended Learning - SIEP Attn: Ebru Altay Oswego, NY 13126 PLEASE NOTE: Applications will not be processed until we receive the $150 deposit.

What do I do next?

You must print out and complete the following documents in order to participate in the program (links to the documents can be found on www.oswego.edu/esl). Please mail the completed forms with your confirmation page and $150 deposit: *Health History and Immunization Form *Meningitis Fact Sheet and Sign Off Form *Insurance and Photo Release Form *Release and Indemnification Form
