Student Transportation Form
The pick up and drop off locations must be in the district's service area.This form must be on file by August 14, 2016, for arrangements to be in place for the start of the 2016-17 school year. Any forms received after August 14, 2016 will be assigned to a previously established bus stop nearest to the pick up/drop off address. If a change in Transportation is necessary during the school year, please complete a new form online and notify your child's building. During the school year, please allow up to 5 days for changes to take place. If your child, 2nd-12th grades should need to ride a different bus occasionally, please send a note to school with your child to be signed by the office and given to the bus driver.
