Please fill out this form completely to register for a time slot to process your completed, unsigned U.S. Passport Form DS-11 (for first-time applicants OR if your last passport was issued before you were 16) or Form DS-82 (for adult renewals).

The Passport Drive will take place at the Office of Global Services (561 LaGuardia Place, 1st Floor) from 10am until 2pm.
