I/We declare the above mentioned child, is allowed to participate in the ROODEPOORT RUGBY CLUB 2019 season, this includes the participation in any competitions, trials, training or events that the above mentioned child is selected for, inclusive of the ROODEPOORT RUGBY CLUB 2019 season. In addition to this, we will not hold ROODEPOORT RUGBY CLUB, nor members or coaches and organisers liable for any damage, loss, or injury sustained by our child or children howsoever arising while he/they participate in the ROODEPOORT RUGBY CLUB 2019 season. This includes any incident related to such participation prior or subsequently to participation.

In this regard, I/We release and discharge ROODEPOORT RUGBY CLUB, nor members or coaches and organisers from all or any claims of whatsoever nature and howsoever arising, whether directly or indirectly, for any damage, loss or injury sustained at any time by our child or children while participating in the ROODEPOORT RUGBY CLUB 2019 season, either in training, tournament, festivals or matches. Any injury sustained by our child or children will be dealt with by the staff to the best of its ability, including any medical attention, the engagement of medical personnel, or the transportation of our child or children to a medical facility. I/We agree to cover all expenses incurred for such medical attention and agree that ROODEPOORT RUGBY CLUB, nor members or coaches and organisers will not be responsible for such expenses.

By signing this form, I/We acknowledge that this release is given irrespective of whether the damage, loss or injury arises from the negligence of ROODEPOORT RUGBY CLUB, nor members or coaches and organisers against none of whom I/We shall have any claim.

By signing this form, I/We also allow our son to be transported by either a parent or coach to and from matches should I/We not be able (or available) to transport our child, or children. Neither the parent, nor the coach/organiser so transporting our child or children will be liable for any damage, loss or injury sustained by our child or children while being so transported. In this regard, it is preferred that transport is arranged by ourselves. ROODEPOORT RUGBY CLUB will not be held liable for any injury or damage to our child before or after pertaining to any event associated with ROODEPOORT RUGBY CLUB.

I hereby consent to the collection and use of personal images of my son by photography or video recording. This pertains to this and any other events arranged or attended by ROODEPOORT RUGBY CLUB. I acknowledge these may be used on the ROODEPOORT RUGBY CLUB website, in social media, newsletters and marketing and promotional material. I further acknowledge that ROODEPOORT RUGBY CLUB may use my image to promote ROODEPOORT RUGBY CLUB and its sponsors in future. I understand that information gathered by ROODEPOORT RUGBY CLUB may be provided to partners which assist the program, I also understand that my consent can be withdrawn at any time in writing to ROODEPOORT RUGBY CLUB.
