Halsted Middle School PTO Family Directory

Our PTO would like to put together a school directory that includes family information. Because of FERPA laws, the school cannot release information for general use. We, the PTO, see this as an opportunity for parents to be able to make connections with other parents and to facilitate our ability to contact families for help. The directory would be published alphabetically as well as by grade level, and would include information such as parent/guardian(s) name(s), child’s name, address, phone number and email address. Only families that complete the form below will be included. Only families that sign up for the directory will receive a copy.
