New Albany - Plain Local School District PHYSICAL EDUCATION WAIVER APPLICATION FORM

This form must be completed and submitted PRIOR to the first day of practice:

All fall sports - August 1st
Girls Basketball - October 28
Boys Basketball - November 4
Bowling - November 4
Hockey - November 4
Swimming - November 4
Wrestling - November 11
Baseball - February 20
Lacrosse - February 20
Softball - February 20
Boys Tennis - March 6
Track and Field - March 6

In accordance with Ohio Revised Code Section 3313.603 and New Albany Plain Local Board of Education Policy 5460, New Albany High School students may be excused from all physical education course requirements by participating in District-sponsored interscholastic athletics, marching band, or cheerleading for at least two full seasons while enrolled in high school and PRIOR TO ENTERING THEIR 12th GRADE YEAR OF SCHOOL. Students entering grade 12 who have not completed the waiver requirements or taken a physical education course will be automatically enrolled in physical education their senior year, EXCEPT FOR THE CLASS OF 2017.

Ohio Revised Code defines District-sponsored interscholastic athletics, marching band, or cheerleading as any high school athletic team, high school cheerleading squad, or high school marching band that participates in the district’s regular athletic season(s) and is operated under the supervision of a Board employee. Club activities and/or club sports are not considered part of the district-sponsored athletics, marching band, or cheerleading.

Students participating in the PE waiver must complete one-half unit (.50 credit) in another course of study which is designated by the Board of Education as meeting the high school curriculum requirements. Such instruction shall be separate from and in addition to all other courses of study and hours of instruction that are required in order to graduate.

Students who wish to use the PE waiver must complete the waiver form prior to the start of the season in which they are participating in the activity. This form will then be verified by the athletic director or band director to document full participation throughout the season.

Procedure for Documenting Physical Education Course Exemption

Prior to participating in a full season of a sport, marching band, or cheerleading, the student must submit a Physical Education Waiver form to the Counseling Office or complete this online form.

Completed forms submitted online will be kept in the Counseling Office until the season is completed. Forms will then be verified by the athletic director or band director.

After completion of the second full season, the school counseling secretary will add the PE Waiver to the student’s official transcript.

The Ohio Revised Code mandates participation in at least two (2) full seasons of an approved sport, cheerleading, or marching band in order to waive the PE requirement. Participation in only one (1) full season cannot be combined with 0.25 credits of PE to meet the PE requirement for graduation.

Submitting this online form does not mean it is automatically accepted. If one of the guidelines above is NOT followed, completing this form is irrelevant and the student should enroll in traditional physical education courses.
