This form will ask a series of questions for students or parents to detail the incident and the individuals involved. It is necessary for students or parents to identify themselves should administration need to ask further questions. Out of respect for the student’s or parent's privacy, all information submitted in this form is strictly confidential and will not be distributed.
Place the completed form in the Behavioral Incident Response mailbox located outside of the 7-12 school office.
