Fleet Management
Please fill out the following information for each vehicle that will be added to your fleet account.
Each vehicle will have its own barcode placed in the windshield and will be billed separately on a monthly basis.
If a vehicle is sold or traded, please notify us as soon as possible so we can remove or replace that vehicle in our system.
After this form has been completed, a member of our Central Office staff will reach out to collect your credit card information which will be kept on file for future billing. This is done during normal office hours, which are Monday-Friday from 8:00 a.m. through 4:00 p.m. Any requests submitted outside of those hours will be completed during the next business day.
