Under the iPad User Agreement, iPad deposits are refundable. Parents of MISD students may use this form to apply for a refund of iPad Deposits and will need to be completed by the September 1st of the following academic school year.

A separate form must be completed for each student. Please allow 8 - 10 weeks for processing.
