IMPORTANT NOTES

- In order to keep the cost of choir membership affordable, we rely on family volunteers (grandparents, siblings, aunts, uncles, etc.) to assist at rehearsals, concerts, and other events throughout the year. In most cases, this commitment is simply a request for a couple hours of your time.

- While we do understand that even a small amount of volunteer effort is a hardship for some families, we also need to ensure that other families are not required to assume a disproportionate share of volunteer responsibilities. For this reason, if you are unable to fulfill your volunteer obligation, we request a waiver fee of $75 (see box below).

- If you have more than one child in the choir, you need only complete this form or pay for waiver once. Enter only one child's name below.

- If you are assigned a volunteer position at concerts, you will still need to purchase a ticket for yourself. MCC would love to offer free admission, but it is not in a position to provide this "perk" at this time.
