Logo Request and Release Form
Committee chairs, board members, and MEHA members in good standing may request the use of the MEHA logo by submission of this form. Request form will be reviewed by a person designated by the Executive Board using a review checklist. If designated person requests additional review from Executive Board, permission may be delayed until after the next scheduled board meeting.
