Incident Report Form
Members of the MECA community must complete the following form for incidents that occur on campus. The Incident Form aids the College in meeting the Jeanne Clery Reporting Act requirements and ensuring the safety of the MECA community.
MECA staff will maintain appropriate confidentiality with each referral.

Provide a detailed description of the incident including: when it occurred, how you became aware of the situation, who was involved, and where it occurred. Remember to be objective, not draw conclusions, and report only your experience (i.e. what you saw and heard).
