Housing Maintenance Service Request Form (Non-Emergency)
General Housing Maintenance
Please fill out this form to help us manage your maintenance requests effectively and allow us to respond to them in a timely manner. Once we have received your request, the maintenance crew or vendors will be assigned or scheduled as soon as possible. Please list one maintenance item per form request.

Notice: Tenants are responsible for minor repairs arising from normal usage or any major repairs/replacements due to gross negligence. For more details, see the school procedure #760: http://wiki.mca.org.tw/doku.php?id=procedures_manual:700_expatriate_personnel.

1. Life safety Emergencies such as fire, please call 119 and call gate guard at 2292-1171 ext. 530
2. Other Emergencies such as no water, electricity, gas odor, broken water pipes*, etc, please call the housing management office or the school main office during school days. After hours, weekends or holidays, please contact housing manager, Amy (0919-836-329) or housing assistant, Serene Brown (0921-638-208).
