LCHS Advanced Placement Testing Registration Form - 2014-15

This form is to register for AP Testing at Lake Central High School for exams that are NOT paid through state and local grants. If all the exams you will take are paid for by grants, you DO NOT need to complete this form. ONLY COMPLETE THIS FORM FOR EXAMS YOU INTEND TO SIT FOR THAT ARE NOT COVERED BY STATE AND LOCAL GRANTS.

By completing this form you are registering and committing to take the AP tests you select. After submitting, you are responsible for all associated fees.

THIS FORM ONLY REGISTERS YOU FOR THE EXAM. You still must pay for the exam(s) you select. The cost for each Advanced Placement exam for the 2014-2015 school year is $91.00 per test (unless covered through state and local grants).

Option 2 - Cash, Check, or Money Order may be dropped off to Mrs. Myers in the LCHS guidance office. If you choose to pay by check or money order, please make payable to Lake Central High School.

Payment Deadline:
Payments for AP exams must be received by March 27, 2015 (unless covered by state and local grants). If a student orders an AP exam and then elects NOT to take the exam, then a $15.00 per exam restocking fee will be charged.
