All products being returned, must be returned to the Lakeshore Veterinary Specialists Glendale location within 2 weeks of original purchase date. Returns will only be accepted for items received in unopened, original packaging and in resalable condition. Prescription drugs are not returnable except for medications in a sealed box. Example: Heartworm and flea and tick preventative. *Shipping and Handling charges are not refundable
Please complete the below online return form within 2 weeks of original purchase. Once the return form is received by an LVS team member, the LVS team member will provide a return authorization number to the client. Clients has the option to ship or physically deliver product(s) to the Lakeshore Veterinary Specialists Glendale location. No shipping and handling charges will be refunded. The client will need to provide product(s) in a damage proof package. Products received in an unsellable condition will not be eligible for a refund. If client would like nonrefundable product back they will be responsible for additional shipping and handling fees. Products need to arrive with a copy of the return form filled out online, along with the return authorization number on the outside of the package. A tracking number must be provided to LVS to ensure proper tracking and responsibility transfer can occur. Once item is received, the client will be contacted by our accounting technician for refund.
