Kindermusik Registration Form
Please complete the following form to register for Kindermusik. READ CAREFULLY BELOW BEFORE ENROLLING!

*All NEW registrations will be charged a one-time registration fee of $20.00

*An invoice will be emailed to you prior to the start of the semester.

*Please note that enrollment is not complete until payment is received.

*Please note that a material fee is part of the cost of every kindermusik class. Everyone will automatically be charged $40.00 for digital access to streaming music and apps, physical materials are offered only upon request.

*Please also note that refunds are not available for missed classes. Partial refunds are available for any canceled classes that we do not make up within the same semester. Make-up classes for missed days are offered upon request IF POSSIBLE, but MUST BE MADE UP WITHIN THE SAME SEMESTER OF ENROLLMENT. Make-Up classes are NOT guaranteed and are handled on a case-by-case basis and offered only if age-appropriate classes within the same semester are not too full. Make-Up classes CANNOT be redeemed outside of the semester of enrollment.

*Each class is 45 minutes long once per week. Please arrive on time to class as it is disruptive to the other participants to arrive late.

*Please keep your child home if s/he is exhibiting any symptoms of contagious illness (fever over 100*, coughing excessively, diarrhea or vomiting, skin rash or any other symptom you would not want your own child exposed to).
