IMPORTANT: This is a quick two-step process; the first step is the payment, the second step is submitting this form.
After you sign and date this form, there is a PayPal link that you will need to click on in order to pay the non-refundable enrollment fee and tuition fee for your selected program and to be officially enrolled. The link will bring you to the PayPal page where you can pay the Non-Refundable Enrollment Fee ($25) and Tuition (amount depending on which program you choose to enroll in). You are not required to have a PayPal account to use this feature. There is a gray button near the bottom of the PayPal page to pay by credit or debit card.
Please DO NOT FORGET to click the SUBMIT button AFTER you have made a payment. Only after these two steps are completed are you officially enrolled.
After payment is complete, you will receive a statement from Mrs. Rebekah Joy, Accounts Receivable Officer; after this enrollment contract form is submitted, you will receive an Information Sheet from the Evening Programs department.
