RSO of the Month Nomination Form
This form is used to nominate RSOs for consideration in selection of the RSO of the Month.  A nomination may be submitted by a member of the RSO being nominated or anyone from the campus community who can accurately report on activities in which the RSO is involved deserving of recognition.  All or a portion of the information provided may be displayed on the OSA website or in other ways to promote the activities of RSO's at IWU.  The information may also be saved for archival purposes and shared with the campus community for years to come.  To provide photos or additional documentation in support of your nomination please send them to osa@iwu.edu.  All RSOs nominated must have submitted their annual RSO registration through the OSA website for the current academic year in order to be eligible for consideration in selection RSO of the Month. Any RSO nominated who has not completed their annual registration will be disqualified from consideration until the annual registration is completed.
