Greetings, Oak Creek Families!

It is time for us to start making plans for the 2017-2018 school year.

To help us determine our staffing needs for the upcoming year, we need to obtain an accurate account of our returning student enrollment. Please fill out and submit the survey below as soon as possible (no later than March 10, 2017).

Please complete one of these surveys for each of your children who currently attend Oak Creek Elementary School. This can be done by selecting "Submit another response" on the confirmation page. Note: these surveys are submitted electronically, and do not need to be printed out.

IUSD is also requiring an updated Residency Verification Form for each student. This form is available to print online, and you will see a link to these forms on the confirmation page at the end of this survey. Please return the completed IUSD Residency Verification Form along with two current proofs of residency and a photocopy of current photo identification (CA driver's license or ID Card, U.S. passport, etc.) to our school's front office by March 10, 2017.
