Transcript Request
For Alumni - To request a copy of your high school transcript, complete the form below. An official transcript will be mailed to the college/business you specify. 

Please note: PEM Schools charges a $2.00 fee for alumni. The fee may be paid by debit/credit card or by check. After you submit this request please go back to the HS Counseling webpage and select Transcript Payment link to pay fee.  Transcript will be mailed out when confirmation of payment is received.
