2013-2014 Semester 2 - Club Sign Up Form
Please fill out a separate form for each of your children who plan to participate in clubs. If you have multiple children participating in clubs, after you click "SEND FORM", you will be taken to a screen with a link that says "SUBMIT ANOTHER RESPONSE". Click that link for a new form. You will need to do this for each of your children.

By clicking "submit form":

1. You are committing that your child(ren) will be at the club for the entire semester (unless you notify the office before noon of that day)
2. You are committing to pay in full by February 3 - fees listed below
3. You are committing to all other extracurricular polices detailed in the Hillel Academy handbook, which can be viewed at http://bit.ly/Hillel_Handbook13_14

Please note: There is a reduced rate for all clubs for students already enrolled in extended day on that day.

Please list below the total amount you owe for clubs. Total payment for Karate, Reptile, Ballet or Clubs is due by February 1st. Payments should be sent to the main Hillel Academy office.
Checks should be made out to "Hillel Academy of Pittsburgh".  Please write "clubs" in memo line. There is an additional fee for credit cards.
