VOCAL APPRENTICESHIP PROGRAM ~ PROGRAM REGISTRATION STEP 1: REGISTRATION – YOU MAY REGISTER UP TO THREE (3) STUDENTS PER HOUSEHOLD ON THIS FORM. NOTE: The Handel and Haydn Society expects that all students are enrolled in their school music programs. Complete the registration form below for the program(s) into which you have been accepted and will be participating. We request that you register by August 1, 2013.

If registering only 1 student: Complete Step 1, Section 1 (items 1 – 34), then scroll down to Step 2 If registering more than one student (up to 3 students per household on this form), complete Step 1, Section 1 (items 1 – 34) and Section 2. In Section 2, you can register 2 additional students who live in the same household. After completing Section 1 and Section 2 (if applicable), proceed to STEP 2
