I request authorization to use the recreational facilities (the “Gym and Studio”) provided by the Qatar Foundation (the “Foundation”) at Georgetown University in Qatar (“GU-Q”).  I acknowledge that my use of the Gym and Studio is expressly conditioned on my agreement to each of the terms of this document.  I acknowledge and agree as follows:

1. Use of the Gym and Studio involves physical exercise and activities that may cause physical injury. I understand that there is an inherent risk of injury when choosing to participate in any physical exercise, wellness or recreational activity, and I agree to assume the full risk of any injuries (including death, illness and other medical conditions), damages or loss which I may sustain as a result of participating in any and all activities arising out of, connected with, or in any way associated with my use of the Gym and Studio.  My use of the Gym and Studio (including group activities and individual use of the facilities or exercise equipment) is a voluntary activity in all respects.

2. I will consult with a physician before I undertake physical exercise in the Gym and Studio. I acknowledge that I have either had a physical examination and have been given my physician’s permission, or that I have decided to use the facilities of the Gym and Studio without the approval of my physician.  I certify that I am in good health and adequate physical condition to properly use the Gym and Studio; that I am knowledgeable about proper use of the equipment or will schedule a gym induction through GU-Q Human Resources to learn about proper use of the equipment and the rules of any activities in which I will participate; and that I will carefully read the operating instructions for any Gym and Studio equipment prior to use and will operate such equipment in strict accordance with such instructions.

3. I acknowledge that neither GU-Q nor the Foundation is responsible for any loss or theft of personal property brought to or left in the Gym and Studio, and I release GU-Q and the Foundation from any liability for such loss or theft.  I have been advised that personal valuable items should be left in a locked unit with the user’s office and not in the Gym and Studio or changing areas.

4. I understand and agree to adhere to the Gym and Studio’s policies and rules, which are displayed in and around the Gym and Studio areas.

5. I will sign in and out of the gym using the register provided, for safety reasons, and if using the facility out of normal office hours, I will inform Security of my intentions.

6. I understand that the Foundation provided the Gym and Studio (and all equipment therein) and that, accordingly, GU-Q assumes no responsibility for the Gym and Studio and its facilities and activities.  I acknowledge that neither GU-Q nor the Foundation can absolutely guarantee my safety in every situation, and that GU-Q and the Foundation have taken reasonable measures to inform me of potential risks to my safety.  I, therefore, agree for myself, my heirs and my personal representatives, to hold harmless and forever release and discharge GU-Q and the Foundation and their respective officers, directors, employees, students and agents from and against all claims, demands, actions or causes of action, deriving from damage to personal property, personal injury or death or any other claim that may arise in connection with my use of the Gym and Studio, except for such damages or injury as may be caused by the gross negligence or willful misconduct of the employees or the agents of GU-Q or the Foundation.  

I have read and fully understand this Acknowledgement and Assumption of Risk set forth above, and I am 16 years old or older.
