DTV Announcement Request Form
The DTV staff will make every effort to accommodate announcement requests that fit our class deadline schedule and are submitted using this form.

If you would like to submit a multimedia element (photos, videos, etc.) to accompany your announcement, please email it to dtv@freedomarea.org, and indicate the announcement with which it belongs.

Only fields marked by red asterisks are required.
